This article provides key information you need to know about utilizing the form Section 504 Snapshot for Annual Review.

  • The recommended timeline to complete this form is 2-4 weeks before the time in which an annual review would be conducted.
  • Based upon the data gathered for the completion of the form, if concerns are noted, the student will receive an annual review meeting. However, if no concerns are noted the form can be completed and a parent letter generated, letting the parent know that no annual review will be held and informing the parent of his/her rights to request a meeting if desired.

  • To use this form, the student must have had a Section 504 Evaluation completed and locked in the application.
  • Required fields on this form include the date the snapshot form was completed, as well as the date the parent was contacted with the results of the snapshot form and the method by which the parent contact was made. The required fields are highlighted in yellow.
  • All areas on the form must be addressed, including Attendance, Grades/Classroom Performance, State Assessment/Benchmark testing, Discipline, and Health/Social-Emotional well-being. You are not allowed to save a draft copy of this form without some information in each area.
  • Based upon the data collected, if it is decided the student does not need an annual review meeting, then the "Date of Snapshot" serves as the date of the annual review upon locking the completed form.

  • The data from the form will print to PDF in a letter template.
  • If the student is missing a current evaluation decision, the letter will not print. This information is obtained from Program Compliance.

  • If this form is not available for you to select in the form drop-down for Section 504, that means that the form has been hidden. Only someone with district administrative permissions can unhide the form by navigating to District Admin > Districts > Edit Customization Options, finding the "Hidden Forms" area, and then clicking the red X corresponding to "SE 504 - Section 504 Snapshot for Annual Review". Then click Save to cause the form to be made visible.

  • To customize shared lists on this form, go to District Admin > Lists > Edit Shared Lists. Then use the "Select Plugin" dropdown list to select "Forms.Section504Snapshot.Plugin".
