Section 504 State Assessments Report

Provides state assessment accommodations for students receiving Section 504 services, based on the last locked Section 504 Student Services Plan form.

Troubleshooting the Section 504 State Assessments Report

To send state testing accommodations to the State Assessments Report, the Student Services Plan form must be locked; we do not obtain data from forms still in draft.

TIP: Generate the Students by Form report for the student services plans to check which documents may still be in draft and need to be locked.

The following table indicates the field value you must choose before generating the "Students by Form" report.

Field Value
Form Name Select "Section 504 Student Services Plan".
Form Lock Status Select "Draft" or "Both".
Start Date You may choose the start date to the beginning of the school year.
End Date You may choose the current date.
Group By Select "Forms".

If the last locked Student Services Plan form occurred in a previous academic year, the accommodations data is also included in the report.

If you have students appearing on this report who are no longer enrolled in the district or are no longer supported through Section 504, it will be helpful for you to ensure that all students who are no longer active in the Section 504 program for your district have been marked "Not Active" on his/her "Program Compliance" tab.

  • This includes students who:
    • Have moved/withdrawn from your district. This requires a manual status change on Program Compliance.
    • A student who is no longer eligible for Section 504 Services. Completing and locking a Section 504 Evaluation with the Evaluation Decision of "Not 504 Eligible" automatically changes the status to Not Active.
    • A student who is no longer receiving Section 504 Services due to eligibility for Special Education. Completing and locking a Section 504 Evaluation with the Evaluation Decision of "IDEA Eligible and 504 Dismissal" automatically changes the status to Not Active.
TIP: You can run a student roster for Active Section 504 students using either the "Student Campuses and Grades" report or the "Student Directory" report and compare this list to a list generated by your Student Information System (SIS). It is best not to wait until the end or beginning of the school year to complete this process. It is recommended that this process be completed routinely throughout the school year.

After the steps described previously are complete, if you still have students you expect to see on the report but do not, this may indicate that one or more students are missing an "Evaluation Decision" as indicated on the "Program Compliance" tab.

Students with no "Evaluation Decision" are missing from the "State Assessments" report. The "Evaluation Decision" field is populated after and from the locking of the Section 504 Evaluation form. If this form is missing or still in draft, this field may be empty.

Tip: Use the Section 504 Compliance report to check for students missing an "Evaluation Decision" and that who should have one.

When selecting the parameters for the Section 504 Compliance report, make sure you choose the "Select All" option at the top of the list in the Evaluation Decisions field.

This section describes what to do in the event your State Testing Accommodations currently available in your district are not up to date.

The State Testing Accommodations shared list is a district-controlled shared list that must be kept current by someone in a district administrative role. For detailed instructions about how to update the shared list, refer to the Changing SE 504 State Test Accommodations article.
