Working Your State Assessment Data (Section 504)

Section 504 State Assessments Report 

Report Triggers:

  • Student has a locked Section 504 Evaluation with an Evaluation Decision indicating the student needs a Service Plan
  • Student has a locked Section 504 Student Services Plan with the State Test Accommodations table completed

Initial Steps

Before running the Section 504 State Tests by Accommodation report:

  • Run the Student by Form report to ensure all draft Section 504 Student Service Plans are locked. Suggested report parameters:
    • Program Statuses: Select All
    • Form Name: SE 504 - Section 504 Student Service Plan
    • Form Lock Status: Draft

Viewing Report Data

If... Then...
The student’s accommodations are grouped in a single text field
(*Test, Subject, Language, and Comments field will be blank.)
  • The student’s last Service Plan was completed prior to March 2016
  • Consider completing a new service plan that allows for individual subjects and accommodations
The student’s name does not appear on the report
  • The “State Test Accommodations required at this time” checkbox is checked on the last locked Services Plan
  • The “Evaluation Decision” field of Program Compliance is empty/blank (populated on lock of the Section 504 Evaluation form)
  • The “Evaluation Decision” indicates the student does not need a Service Plan. - changes have been made to the district customizable shared lists that removed the original selections
The student’s name appears, but some of their tests are missing
  • Changes have been made to the district customizable shared lists that removed the original selections
