If your student is being dismissed from Section 504, has been evaluated by and is now eligible for Special Education, the steps below will walk you through the proper way to address it in your Section 504 Evaluation form.
Go to SE 504 and enter in a few characters of the students name in the "Enter Students Last Name..." dropdown to locate the student in question. You can then add the Section 504 Evaluation form from the "New/Edit Forms" dropdown.
This selection will allow you to mark the evaluation as Re-Evaluation under the Type of Evaluation.
If the student is being dismissed from Section 504 and is being evaluated by Special Education and is eligible, click the option under Section 504 Committee’s Decision that says IDEA Eligible & 504 Dismissal. Save and lock the form. Upon lock of the form, the program will automatically change the student’s status on Program Compliance to "Not Active."
