If a student is being dismissed from Section 504, you must document the dismissal in a “Section 504 Evaluation” form. This process ensures the student’s eligibility decision is properly recorded and automatically updates the student’s Program Compliance status.
Follow the steps below to complete a Section 504 dismissal correctly.
Step 1: Locate the Student in “SE 504”
- Navigate to SE 504.
- In the Enter Student’s Last Name… dropdown, type at least a few characters of the student’s last name.
- Select the student from the list.
- From the “New/Edit Forms” dropdown, select Section 504 Evaluation.
Step 2: Select Re-Evaluation
On the “Section 504 Evaluation” form:
- Under “Type of Evaluation,” select Re-Evaluation.
This ensures the evaluation reflects an updated committee decision rather than an initial eligibility review.
Step 3: Select Dismissal from 504
Within the “Section 504 Committee’s Decision” section:
- Select Dismissal from 504.
- Click Save.
- Click Lock to finalize the form.
What Happens After You Lock the Form?
When the “Section 504 Evaluation” form is locked with "Dismissal from 504" selected:
- The system automatically updates the student’s Program Compliance status to "Not Active."
- The student will no longer appear as an active Section 504 student.
- The dismissal is documented for reporting and compliance purposes.
