SPP Indicator 11

SPP Indicator 11 Triggers:

The Notice of Evaluation form has been completed and saved.  
  • A date has been entered into ‘The district received a request to evaluate the student for initial special education eligibility on’ field.
 
  • Yes has been selected.
 
  • A selection has been made in the ‘If Yes, the student was referred for a special education evaluation by’ field.

The Consent for Evaluation form has been completed and saved.  
  • Type of Evaluation has been marked as Initial.
 
  • A date has been entered into Signature Date field to the right of the Signature of Parent/Guardian/Adult Student at the bottom of the form.

The Full and Individual Evaluation Form is one of three triggers for SPP Indicator 11  
  • Full Individual and Initial Evaluation (FIIE) should be used as the Report Title.
 
  • FIIE is the default Report Title when a student’s status is Referral.
 
  • The FIIE must be locked.
 

Before running your SPP Indicator 11 Report:

If the district has opted to use a different Consent for Evaluation form, the Initial Consent for Evaluation date may be manually entered on Program Compliance.

  • Run a Custom Report. Include Initial Consent for Evaluation field and Special Education Status.
      • Export out to Excel and filter down to just Initials and Status DNQs.
  • Run the PEIMS 163 History Report, for students with a Program Status of Initial to give you a list of students from the current year.
      • Note: DNQ does not create a PEIMS 163 record. If the district has opted to use a different Consent for Evaluation form, the Initial Consent for Evaluation date may be manually entered on Program Compliance.

Run the Students by Form report to ensure all draft FIES are locked.

  • Suggested report parameters:
      • Program Statuses: Referral, DNQ, and Initial.
      • Form Name: SE FIE - Full and Individual Evaluation V2 and Full and Individual Evaluation Upload.
      • Form Lock Status: Draft.

If the district has opted to use a different FIE document:

  • then SPP Indicator 11 will not display student; or
  • the Full and Individual Evaluation form may be locked with just the Date, Report Title: Full Individual and Initial Evaluation, and Parent Name fields

When viewing the SPP Indicator 11 report data:

If... Then...
a student is missing an Initial Consent for Evaluation date
  • there is no saved parent Signature Date on the Consent for Evaluation form; or
  • consent date was not manually entered on Program Compliance
the student has a locked FIE, but does not appear on the report
  • ensure the title of the FIE is saved and locked as Full Individual and Initial Evaluation;
  • ensure there is a check in the box for ‘The district received a request to evaluate the student for special education eligibility on’ and a date have been entered into the date field;
  • ensure there is a check in the box for ‘The student was referred for a special education evaluation by’ and that a selection was made from the dropdown (e.g., parent, school, other).
